Referral for Psychotherapy at Paris Mind Clinic
At Paris Mind Clinic, we partner with physicians and nurse practitioners to make the referral process for psychotherapy services in Ontario simple and efficient. To facilitate a smooth experience, we offer a downloadable referral form that captures all necessary information, helping us provide prompt and effective mental health support. Alternatively, physicians may use their own office’s referral form if preferred.
Instructions for Completing the Psychotherapy Referral Form
For referring professionals (physicians and nurse practitioners):
- Download the Referral Form: Click here to download the referral form if you choose to use our clinic’s form.
- Complete and Sign: Fill in the necessary details such as the patient’s name, date of birth, health card number, and your contact information. Specify the reason for consultation by selecting appropriate options like Depression, Anxiety, PTSD, etc., or provide a DSM-5 diagnosis for other conditions.
- Sign and Submit: After completing the form, please sign it to confirm the referral, and choose to either fax it to 647-493-2584 or email it to info@parismindclinic.ca.
Note for Referring Professionals: You may use the referral form provided by our clinic or your own standardized form. We accept both to accommodate your workflow preferences.
